WHAT TO EXPECT FOR YOUR CHILD'S
ELIGIBILITY EVALUATION
PARENT INTERVIEW
Our team performs parent interviews over the phone or in-person using an assessment tool called the
Routines-Based Assessment Tool.
Questions include:

What is your family medical history?
-
What is your child's birth/medical history?
-
What is your child's current weight/height
-
Questions about your child's nutrition, including what and how they eat.

Questions about your child's daily routines (i.e. How does your day begin? How does your child participate in daily routines? How much does your child do for him/herself? How is your child interacting w/others?)
-
What are your child's interests?
-
What are your child's/family's strengths?
-
What are your main concerns?

IN-PERSON EVALUATION
Our team uses hands-on evaluation, observation, as well as a tool called the TPBA (Trans-Disciplinary Play Based Assessment) to determine a child's eligibility.
A child is eligible to receive Tiny-K early intervention services if:

The child is automatically eligible due to established risk for developmental delay.

The child demonstrates a developmental delay of 25% in one developmental area or a 20% delay in 2 or more developmental areas.
-
The child is deemed eligible to receive services based upon evaluators informed clinical opinion.
